is a year-long Lifestyle Change Program. It is for people at high risk for developing type 2 diabetes. This program focuses on changes in diet and increased physical activity. YMCAs also offer a
version of this program

The CCDPC supports providers of diabetes self-management education. Examples of these include
programs provided by Maryland's Aging Network, and hospital based diabetes self-management education.

Providers of the Diabetes Prevention Program, Diabetes Self-Management Education, and the Living Well Chronic Disease Self-Management Program add information to keep the site up to date with current classes throughout the state.
