Diabetes Prevention Program (DPP)

The evidence based National Diabetes Prevention Program is a year-long Lifestyle Change Program. It is for people at high risk for developing type 2 diabetes. This program focuses on changes in diet and increased physical activity. YMCAs also offer a version of this program.
 
 

Diabetes Self-Management Education Programs

The CCDPC supports providers of diabetes self-management education. Examples of these include Living Well programs provided by Maryland's Aging Network, and hospital based diabetes self-management education.
 
 

Diabetes and Prevention Classes- Diabetes and Prevention Classes

The CCDPC has created Be Healthy Maryland website, a one-stop site with information about a variety of chronic disease, diabetes, and diabetes prevention programs that are held throughout the state. It is available to anyone who wishes to find classes, and to medical professionals to make referrals for their patients. Providers of the Diabetes Prevention Program, Diabetes Self-Management Education, and the Living Well Chronic Disease Self-Management Program add information to keep the site up to date with current classes throughout the state.
 
 

Healthiest Maryland Businesses (HMB)

Healthiest Maryland Businesses is a partnership among Maryland businesses that increases wellness within workplaces. Participating businesses are referred to accredited work site wellness resources, receive education and assistance, and are recognized for their commitment to improving the health of their workforce.
